Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) There were no automatic alarms on Reactor 6B to alert employees to evacuate if a significant increase in temperature or pressure occurred in the northwest corner of the main production building. A feasible and useful method to correct this hazard among other methods, is to install alarms to activate with increased temperature and/or pressure.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) There was no established safety procedure of written safety instructions to operators of Reactor 6B when increased pressure occurred in the northwest corner or the Production Building. A feasible and usefule method to correct this hazard among other methods is to develop written safety procedures and instructions to address an increase in pressure in the reactor system.

A feasible and useful method to correct this hazard among other methods is to obtain a certification from the Ohio Division of Boiler Inspection.
