Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of the activation of moving parts in the equipment which if
energized, could expose the employees to contact with rotation blades
in the blenders/steamser:
a) On July 25, 1989, employees were exposed to moving parts in the
blenders/steamers 1 through 6 in the Ready-to-Eat Room while having
to lean down into the 42 inch high blenders/steamers which have
blades that rotate about 12 inches below the tops. A hydrochloric
acid solution with a ph of 0-1 was used to clean the blenders/
steamers. Among other methods, one feasible and acceptable abatement
method to correct this is the establishment of a lockout program
consisting of the following minimum safe work practices.
1) Before any maintenance, inspection, cleaning, adjusting or servicing
or equipment (electrica, mechanical or other) that requires entrance
into or close contact with the machinery or equipment, the main power
disconnect switches or valves, or both, controlling its source of
power or flow if material shall be locked out or blocked off with
packlock, blank flange, or similar devices. Residual energy in
equipment having systems such as pneumatic, hydraulic, compressed
springs, or other shall have these systems bled off, blocked or other
means implemented to ensure that all sources of energy have been
deactivated prior to work commencing. However, it must be verified
that the equipment has been rendered safe (either by use of
electrical test equipment or by attemp to activate thh equipment).
2) Locks keyed spearately to ensure removal only by the authorized
person shall be provided to each person working in areas requiring a
lockout. Where more than one person is involved in a single operation
or where multiple operations such as both electrical and mechanical
are being simultaneously performed on a single piece of equipment,
locks shall be used by each person.
3) A written lockout policy shall be established including procedures
for locking out equipment or machinery. Employees involved in
implementing the program shall be trained in all elements thereof.
4) There shall be regular enforcement and periodic evaluation of the
program to ensure usage and adequacy of the program.
