Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees in the walkway area at Elevation 229 between Columns H6 & H7 were exposed to the hazard of being crushed by a falling plat- form due to passing beneath a platform while it was suspended overhead by chain hoists and wire rope slings. (b) Employees in the walkway area at Elevation 229 between Column lines J6-J7 and H6-H7 were exposed to the hazard of being crushed by a falling steam blow pipe due to passing beneath the pipe while it was suspended by chain hoists and wire rope slings and a web sling. Among other methods, feasible and acceptable methods to correct this hazard are the blocking or cribbing of the suspended load or the pre- tection.
