Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On or about the morning of 4-18-00 employees working with, or in the
vicinity of, the #1 Horne
machine ( New Britain Model 80 Grinder ) were exposed to the hazard of
being struck by
fragments of an exploding grinding wheel in that a used grinding wheel
that showed evidence
of damage was mounted in the machine and run.
Among others, one feasible method of abatement would be, in accordance
with ANSI (American
National Standards Institute ) B7.1 "safety requirements for the use,
care, and protection of
abrasive wheels", to ensure that following inspection, grinding wheels
which show evidence of
cracks or other damage are not mounted in grinding machines.
