Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

were exposed to:
falls in excess of 6', in that on or about 10-12-2000 employees were
lifted from containers by
use of a nylon sling attached to the hook of the Harrington hoist to enter
and exit the steel
containers.
One feasible and acceptable abatement method to correct this hazard is to
develop and adopt
rules and procedures which are consistent with ANSI B30.16 (which
prohibits the lifting of
personnel) and to include the use of ladders or stairs to enter and exit
containers.

employees operating the Harrington hoist were exposed to hazards
associated with electrical
shocks and being struck by falling loads due to the undetected defects in
the equipment:
a) On or about 10-12-2000 periodic and frequent inspection were not
performed to  determine
the hoist condition and any defects such as but not limited pendant
control strain relief within
the hoist system and supportive parts.
b) On or about 10-12-2000 defects such as but not limited to pendant
control strain relief were
not reported to the designated person for repair prior to use.
c) On or about 10-12-2000 pendant controls were not equipped with a strain
relief to prevent
exposing electrical parts from the motor control.
Among other methods, one feasible and acceptable abatement method to
correct these hazards
is to develop and adopt written standard operation procedures which are
consistent with ANSI
B30.16 and include, inspections, recorded documentation and defect
reporting methods.g
