free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
Employees working in the area were exposed to being struck by a
material handling lift truck because the Lull, Model 400-B, serial
2127 NTF had an obstructed view to the rear.
Among other methods, feasible and acceptable methods to correct this
hazard would be to:
1) Install a reverse signal alarm to bring the lift truck up to its
original manufactured condition; or
2) Provide a signal person or observer to direct the lift truck operator
when it is safe to back up.
