Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a) On or about 12/7/90 employees in the molding area were
exposed to the hazard of being crushed by being struck by the upper
halves of the molds such as the "Y body resin transfer mold" due to
being under the molds while they were suspended from hoists such as
the Dayton 2-ton No. 12 hoists.
(b) Employees operating he Marquis extension molds, Marquis
garnish molds, radiator fan shroud molds, and Buick Reatta mold were
exposed to the hazards of being crushed by falling upper mold halves
while working beneath the molds which were suspended from overhead
hoists.
Among other methods, feasible and acceptable methods to correct this
hazard are the redesign of the molds to eliminate the need to be beneath
them or the addition of a "safety bar" or other device which would
secure the upper die and prevent its falling or being lowered onto an
employee.

of the hoists or their components including hoist chains and hooks.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is the extablishment of a frequent inspection
program, lubrication program, and preventative maintenance inspection
of hooks and load bearing chains and lubrication of all moving parts
for which lubrication is specified (including load bearing chains).

Hoists" or ANSI/ASME B30.20 - 1985, entitled "Below-the-Hook Lifting
Devices."
