Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) On or before February 9, 1994 employee(s) were exposed to the
hazards of falling
loads and/or falling hoist/crane components.  The trolley for the outside
jib crane/hoist was
overloaded as it had a rated capacity of 2 tons and loads of 6,000 to
9,000 lbs. were routinely
lifted. The jib crane/hoist outside and the A frame crane/hoist in the
shop did not have a load
test performed on it before putting it into service.  There was no
frequent inspections performed
by a designated person on the jib crane/hoist located outside.  There was
no marking of the
capacity on the jib of the crane/hoist located outside nor on the beam of
the A frame crane/hoist
in the shop.
Feasible and acceptable means to abate the hazard is to do the following
in accordance with
ANSI B30.16 - 1973:
a)Perform a load test on the jib crane and hoist and the A frame
crane/hoist.
b)Conduct frequent inspections of the hoist and crane by a designated
employee.
c)Mark the safe lifting capacity of the crane, hoist, and trolley or
whichever is the
least, on the jib of the crane, making sure that the lowest rated
component has a
rated capacity for the maximum load lifted and so it's readily visible.
d)Lift only loads within the rated capacity of the lifting
