Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazard that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
The hazards of being seriously injured due to being struck by
flying drilling mud and its associated high pressure piping from
failed connections in the mud supply line running from the Ideco
model 55 duplex mud pump to the drilling rig located at the Rosa Fyke
number twenty-four (24) wellstie, located in Section 8, Township 1
1 North, Range 2 East, Marion County (near Salem, IL).
Among other methods, one feasile and acceptable abatement method to
correct this hazard is to provide a positive means of securing the ends
of all flexible mudline connections with a safety chain or wire rope
during drilling operations, as required by paragraph 8.14.3 of RP 54
Recommended Practices for Occupational Safety and Health for Oil and Gas
Well Drilling and Servicing Operations, and to insure that such means
are properly installed following drilling rig setup and before the mud
pump is placed into operation.
DISCLAIMERS:
1. The employer is not limited to the abatement methods suggested by
OSHA;
2. The methods explained are general and may not be effective in all
cases; and
3. The employer is responsible for selecting and carrying out an
appropriate abatement method.
2) By prohibiting all non-foot traffic, including manually propelled
work boxes and other such vehicles, over all void area in the unit
where the concrete slab measures six (6) inches or greater in depth,
where it has been established that the soil beneath the slab has
greater than a two (2) inch void, and where it has been established
that the span of the void is five (5) feet or less in the least
dimension. In such instances where it has been establishedthat the
span of the void is greater than five (5) feet in the least dimension
means such as that described in item 1 above, shall be employed to
abate the hazard.
ABATEMENT PLAN:
20 Days - Provide supplemental support means in all undermined areas as
detailed above in items 1 and 2.
90 Days - Complete abatement of violations.
