Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likley to cause death
or serious physical harm to employees in that employees were exposed to:
...of being caught by collapsing sections of pavement during routine
operating and maintenance activities in the Reactors Section of the
Alkylation Department, due to failure of the pavement under load over
area wher leaks into or out of teh industrial sewer system had
undermined the supporting fill and/or soil.
Among other methods, one feasible and acceptable abatement method to
correct this hazard (without and immediate rebuilding of the pavement
and fill of the undermined areas) while still maintaining normal
operations in the unit is to restrict the loading on the undermined
portions of the pavement as detailed in the engineering study completed
on January 4, 1990 by a registered structural engineer employed by the
Bechtel Corporation (and attached to this document through such means
as the following:
1) By providing an alternative walking/working surface over all void
areas in the unit sufficient to carry all anticipated loads where
the concrete slab measures less than six (6) inches in depth, and
where it has been established that the soil beneath the slab has
greater than a two (2) inch void, and;
