Section 5 (a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likley to cause death
or serious physical harm to employees in that employees were exposed to:
...various serious injuries due to drilling rig accidents on the
number five drilling rig located at the Hahn number one wellsite,
located in Section 16, Township 10 North, Range 8 East, Cumberland
County (near Toledo, Il), without a means located at the wellsite
to promptly summon emergency medical attention to the accident site.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to provide the means to arrange for any necessary
medical or first aid treatment, such as radiotelephone sets or cellular
telephone equipment, as required under the provisions of paragraph 3.1.1
of API RP 54 Recommended Practices for Occupational Safety and Health
for Oil and Gas Well drilling and Servicing Operations.

...the hazards of being seriously injured due to being struck by the
hopper, piping and hoses of the hopper-type mud mixer attached to the
Emsco Model D375 Mud Pump, attached to the number five drilling rig
located at the Hahn number one wellsite, located in Section 16,
Township 10 North, Range 8 East, Cumberland County (near Toledo, Il).
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to provide a positive means of securing the mixer
and its associated hoses and piping in place either by staking or by
other equivalent means, and to insure that such means are properly
installed following rig setup and before the mixer can be placed into
operation, as required under the provisions of paragraph 8.14.3 of
API RP 54 Recommended Practices for Occupational Safety and Health for
Oil and Gas Well Drilling and Servicing Operations.

previously cited for a violation of this Occupational Safety and
Health Safety and Health Standard or its equivalent standard,
paragraph 8.14.3 of API RP 54 Recommended Practiced for Occupational
Safety and Health for Oil and Gas Well Drilling and Servicing Operations
which was contained in OSHA Inspection Number 2508885, Citation Number
01, Item Number 6, issued on April 28, 1988.

or serious physical harm to employees in that employees were exposed to:
...the hazards of being seriously injured due to being struck by
flying drilling mud and its associated high pressure piping from
failed connections in the mud supply line running from the Emco
duplex mud pump to the number 5 drilling rig on the Hahn number one
wellsite, located in Section 16, Township 10 North, Range 8 East,
Cumberland County (near Toledo, Il).
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to provide a positive means of securing the end
of all flexible mudline connections with a safety chain or wire rope
during drilling operations, as required by paragraph 8.14.3 of AP RP 54
Recommended Practices for Occupational Safety and Health for OIl and Gas
Well Drilling and Servicing Operations, and to insure that such means
are properly installed following drilling rig setup and before the mud
is placed into operation.

Drilling and Servicing Operations which was contained in OSHA
Inspection Number 2508885, Citation Number 01, Item Number 5, issued
on April 28, 1988.
Section (5)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
...the hazards of being burned due to being trapped on the
fingerboard on the number five drilling rig located at the Hahn
number one wellsite, located Section 16, Township 10 North, Range 8
East, Cumberland (near Toledo, Il), during a rig fire without having
a means of escape.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is the installation of an emergency escape means
running from fingerboard to a location remote from the wellhole and the
drilling floor following rigging up of the rig mast, as specified by
paragraph 4.7.1 of API RP 54 Recommended Practices for Occupational
Safety and Health for Oil and Gas Well Drilling and Servicing
Operations.
