General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to haz- ard(s) of: being caught between the elevator floor and the wall opening structure or falling into the elevator pit while the freight elevator was operated with the hoistway gates in the open position. Among other methods of abatement, one feasible means of correcting thi s condition is to install interlocking type gate guards in accordance with Rule 110 of ANSI A17.1-1987, Safety Code for Elevators and Esca- lators. Such provisions prohibit the elevator from operating until the hoistway gates or doors are in the closed position and such gates will not open until the elevator is at the same elevation (level).
