Section 5(a)(1) of the Occupational Safety and Health Act of
1970:  The employer did not furnish employment and a place of
employment which were free from recognized hazards that were
causing or likely ot cause death or serious physical harm to
employees in that employees were exposed to the hazard being
struck by objects that were propelled through the air by an
exploding sand blasting hopper that was pressurized with
compressed air:
On or about August 4, 1993, employee(s) were exposed to tha
hazard of an exploding sand blasting hopper that was being
used to transfer roofing granules to the roof of the Bull-X
plant located in Farmer City, IL.  No safety relief devices
were provided on the pressurized hopper and the compressed
air inlet line was not regulated allowing up to 90 p.s.i. to
be inside the hopper.  The integrity of the hopper had not
been determined by hydrostatic testing prior to pressurizing
the hopper.
Among other methods, feasible and acceptable abatement
methods to correct this hazard would be to:  (1) provide
safety relief devices on the hopper; (2) regulate the
compressed air entering the hopper to a pressure that is
determined safe by hydrostatic testing of the hopper; (3)
Hydrostatic test the hopper at regular intervals and remove
the hopper from service should it not pass the test and, (4)
conduct an engineering study on the method used to apply
granules and institute all recommendations developed from
the study concerning the safety of the system.
