Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Employees are permitted to re-enter and clean conveyor path of travel
at scalding machine while same is in motion with no effective Lockout
procedure in effect to insure a Zero Mechanical Energy State (ZMS)as
evidenced by the following findings:
1) Employee injured after be re-entered scalding machines cycle of
operation while performing a cleaning function with conveyor in
motion.
2) Employees were not aware of any specific verbal or written policy to
govern a routine procedure of cleaning scalding machine.
3) Employee enters scalding machine to clean with system off but not
locked out to prevent unauthorized re-start.
4) Hose flush down of scalding machine permitted while conveyor is in
operation.
5) No lockout procedure training provided for this job function
subsequent to employee injury on January 20th, 1987, to prevent
recurrence other than written warning to employee.
6) Employers safety effort did not effectively eliminate the root cause
of incident on January 20, 1987 5:15 p.m.
7) Written lockout training procedures now being formulated.
A feasible abatement method to correct this hazard is to implement a
written effective lockout procedure by training employees to safely
perform their job functions with regard to moving machinery.
8) Control station is not located so operation of equipment is visible
from them per A.N.S.I. B20.1-1976 sec. 5.11.2.1 nor there warning
device actuated by the controller as specified in Sec. 5.11.2.2
