Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which was free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed:
(a)  Clark Township Road 152, Holmes County:  On or about October 16,
1993, due to
the lack of maintenance, the wire rope used to support the racking board
did not have substantial
strength to carry the weight of employees on the racking board, thereby
resulting in an employee
falling 25 feet to his death.  Among the feasible means of abatement that
could be used to
correct this are:  1)  Daily inspections of all wire rope cables on the
service rig.
2)  Weekly preventive maintenance of the service rig to include detailed
inspection of wire ropes
and full extension of the mast.  This would also include greasing of all
joints and chives with
the results documented and retained for the life of the service rig.  3)
Records kept of all
maintenance done and of any changes made to the service rig.  Also, a
yearly inspection of the
entire service rig done by an outside agency, specifically an agency or
manufacturer who is
familiar with oil and gas service rigs.  Any changes should be approved
by the manufacturer and
the approval kept for the life of the service
