Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
severe burns, asphyxiation, concussions, multiple traumatic injuries
and/or death from the increased likelihood of fire, explosion, and/or
uncontrolled release of natural gas due to improperly manufactured,
supported, constructed and/or maintained natural gas heaters and
supply lines. Specific deficiencies noted at the time of the inspection
included:
a) In the Fabrication Department, near the Do-All metal cutting band
saw, an unapproved shop-made natural gas heater with an unregulated
gas torch and crude sheet metal enclosure.
b) In the Fabrication Department, near the "Koick" burning table,
an unapproved shop-made natural gas heater with an unregulated
gas torch and a crude sheet metal enclosure.
c) The following natural gas-fired radiant heaters were hanging from
unsecured and unstable pipe or angle iron stands by means of
bits of wire or chain at heights of 1 to 4 feet above the plant
floor.
1) 900 Fixture Station, East Aisle.
2) Sub-Assembly Area, Southwest Corner.
3) Corner post, 603-604-605 Area.
4) Beam-Cutting Area.
5) Fit-Up Pontoon Area, Northeast Aisle.
6) 545 Weld-Out Station.
d) The following natural gas infrared heaters were subject to being
struck by pedestrians, cranes lifting and moving sub-assemblies,
and forklift trucks due to their location immediately adjacent to
aisleways:
1) Ray-Tec Infrared Space Heater, 900 Fixture Station.
2) Ray-Tec Infrared Space Heater, Sub-Assembly Area.
3) Ray-Tec Infrared Space Heater, Corner of Station 603-604-605.
4) Ray-Tec-Infrared Space Heater, Beam-Cutting Area.
5) Ray-Tec Infrared Space Heater, Fit-Up Pontoon Area.
6) Ray-Tec Infrered Space Heater, 545 Weld-Out Station.
e) The following flexible hose connectors, supplying natural gas from
column-mounted manifold piping outlets to floor-mounted radiant gas
heaters, were subject to physical damage as they were lying in and
on the aisle floors; lying on walking and working surface of work
stations; coiled around and tangled with 125-volt electric extensions
control and equipment supply cords; arc welding lead and ground
cables; 480 volt, 3 phase electric supply cables for numerous welding
machines; high-pressure hydraulic hoses; sharp-edged steel plates;
scrap, tools, and misc. debris:
1) 900 Fixture Station, East Aisle.
2) Sub-Assembly Area, Southwest Corner.
3) Corner Post, 603-604-605 Area.
4) Beam-Cutting Area.
5) Fit-Up Pontoon Area, Northeast Aisle.
6) 545 Weld-Out Station.
Among other methods, one feasible and acceptable method to correct this
hazard is to assure all natural gas-fired heaters and supply lines are
designed, manufactured, constructed, and maintained in accordance with
the requirements contained in ANSI/NFPA Z233.1-1988, The Natural fuel
Gas Code. Essential elements include, but are not necessarily limited
to, the following:
1) Assure that all gas appliances, gas utilization equipment, and
accessories are approved by a nationally-recognized testing
laboratory, as stipulated in Part 5, Paragraph 5.11 of the
National Fuel Gas Code.
1) 900 Fixture Station, East Aisle.
2) Sub-Assembly Area, Southwest Corner.
3) Corner Post, 603-604-605 Area.
4) Beam-Cutting Area.
5) Fit-Up Pontoon Area, Northeast Aisle.
6) 545 Weld-Out Station.
Among other methods, one feasible and acceptable method to correct this
hazard is to assure all natural gas-fired heaters and supply lines are
designed, manufactured, constructed, and maintained in accordance with
the requirements contained in ANSI/NFPA Z233.1-1988, The Natural fuel
Gas Code. Essential elements include, but are not necessarily limited
to, the following:
1) Assure that all gas appliances, gas utilization equipment, and
accessories are approved by a nationally-recognized testing
laboratory, as stipulated in Part 5, Paragraph 5.11 of the
National Fuel Gas Code.
2) Suspended-type infrared heaters shall be safely and adequately
fixed in position independent of gas and electric supply lines,
as stipulated in part 6.18.1 of the National Fuel Gas Code.
3) Suitable guards of bumber plates shall be installed to protect
the equipment from damage when it is necessary to locate gas
utilization equipment close to passagewys traveled by trucks
or cranes, as stipulated in Part 5, Paragraph 5.1.13 of the
National Fire Gas Code.
4) Protect flexible gas connectors against thermal or physical
damage, as stipulated in Part 5, Paragraph 5.5.3.d of the
National Fire Gas Code.
