Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to the hazard of back injuries, pinch point injuries to fingers/hands/feet, concussions and/or multiple traumatic injuries during the loading, maneuvering and the unloading of freight crates and pallets of material on the fixed dollies and the static racks (roller conveyors) at the El Al Israel, Airline security cage (inside staging) resulting from the employees walking on the static racks (roller conveyors) and the dollies rollers (caters) to maneuver (push)) freight pallet inside the security cage during the loading (staging of pallets) and the unloading of pallets from security cage. Walkways or other means to maneuver the freight pallets inside the security cage were not provided between the dollies and the static racks (roller conveyors) system. Among other methods, one feasible and acceptable abatement method to correct this hazard is to conform to the American Society of Mechanical Engineers and American National Standard, ANSI/ASME B20.1-1990 Safety Standard for conveyors and related equipment, Section 6.11 Requirements for Roller and Wheel conveyors and the National Safety and Council Data Sheet 1- 528-REAF.85 Requirements for roller conveyors and to include, but not limited to the following: 1. (a) Provide walkways between the static racks (roller conveyors) and the fixed dollies by changing the present layout. (b) Design and install a new roller conveyor system. (c) Change the width and/or length of the present cage. 2. (a) Design and install a new security cage layout with belt or chain live roller conveyors system or mobile conveyor system. (b) Close the present cage gate at the middle and provide two new gates at the front and back to allow pallets to be moved in and out at both gates. 3. (a) Remove all roller conveyors system (static racks and dollies) inside the security cage. (b) Close the present cage gate in the middle and provide two new gates at the front and the back to be able to move in and out of the security cage with dollies. (c) Change the location of the present cage to allow equipment to move in and out. (d) Change the present cage width and length to provide walkways between dollies column, if two dollies columns were used. (e) Freight pallets will remain on dollies. (g) Use a tug or other equipment to move dollies in (during staging) and out of the cage.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to the hazard of back injuries and pinch points injuries to fingers/hands/feet during the loading, maneuvering and the unloading of freight crates and pallets of material on the fixed dollies with broken casters/rollers and on the static racks (roller conveyors) at the El Al Israel Air Line security cage (inside staging) and the outside activity. Among other methods, one feasible and acceptable abatement method to correct this hazard is to conform to the American Society of mechanical of mechanical engineers and American National Standard ANSI/ASME 1320.1-1990 Safety Standard for conveyors and related equipment, operation and maintenance requirements section 5.2, 5.12 and 6.11 for roller and wheel conveyors and to include but not limited to the following: 1. Establish inspection and prevention maintenance procedures for all dollies, roller conveyors and other equipment. a.) Assure all dollies and other equipment are inspected before use. b.) Repair and/or replace dollies and other equipment (casters/rollers, etc.) on a planned schedule. c.) Maintenance, such as lubrication and adjustment, shall be performed by qualified and trained personnel. d.) Rollers shall be free running to prevent unintentional diverting of unit, being conveyed when such a diverted unit could create a hazard to personnel. e.) Tag all damaged dollies and other equipment to holding areas to be repaired and/or replaced. 2. Establish normal operating and safety procedures. 3. Instruct and train all affected supervisory and operation personnel in these procedures. 4. Institute a monitoring system to assure compliance.
