Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
a) Employee engaged in a window cleaning function was using a hang
ladder from the south exterior wall of building located at 1233 North
Mayfair Road, Milwaukee, Wisconsin.  No fall protection was utilized
by the employer and on October 22, 1987 at approximately 12:00 p.m.
subject ladder failed causing employee to fall 25 to 30 feet to
ground elevation sustaining injury.
A feasible and acceptable method of abatement would be the enforce usage
of a safety harness life line system or the elimination of hang ladders
which are not sanctioned for usage in window cleaning operations per
American National Standard Institute A39.1-1969, Section 7.1.11 entitled
"Safety Requirements for Window Cleaning".
