Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of being caught between the elevator floor and bottom of the elevator pit or falling into the elevator pit due to the employer not providing complete operational doors or gates or gates with interlocking devices on the personnel freight elevator located on the 1st floor of the plant. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the recommendation of ANSI A17.1-1971, Rule 110.1 of Section 110 to install an interlocking gate or door barrier on the personnel/freight elevator located in grain elevator #4. b) The freight elevator used by personnel for the 8 floors of elevator #4 was not being inspected on a yearly basis to determine the wear on such parts as ropes, bearings, gears and brakes. One rope broke on 3-22-93, allowing the elevator to fall approximately 20 ft. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the recommendation of ANSI A17.1-1971, Rule 1001-1A-2 of Section 1001 to provide an inspection and test program, done on a yearly basis, to check the various safety components of the freight elevator such as the wire ropes, gears, bearing and brakes as well as the counterweights.
