Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On and prior to 4/14/93, workers were exposed to the hazards of severe
cuts, broken bones, amputations and/or death while performing work duties
on slitters at the addressed site.  The noted hazards were created by
moving flat rolled steel traveling from the steel coil payoff mandrel to
the steel slitter, and onto the rewind mandrel.
The hazards can be abated by, but not limited to, a program including the
following criteria covered in American National Standard for Machine Tools
--Coil-Slitting Machine Systems--Safety Requirements for Construction,
Care and Use, ANSI B11.14--1983.
(a)  For small 24" Torrington slitter, barricade employees acess fully
on north and east sides to traveling rolled steel, traveling cut steel
ribbons and traveling cut steel scrap.  Barricade must be of a permanent
type such as fencing, cable or rails as per ANSI B11.14--1983, Section
5.3.1.6.
(b)  For small 24" slitter, furnish jog control to minimize unintended
continuous operation during operations such as threading per ANSI
B11.14--1983, Sections 4.4.4 and B2.1(1).
(c)  Written instructions by manufacturer, modifier or reconstructor to
establish guidelines for care, use, design capacity and maintenance of
the two slitters on site were not available per ANSI B11.4--1983, Section
4.6.  For abatement, obtain original instructions if available, or
substitute equivalent information in current programs.  Specifically, for
securement of turnstile for 24" slitter, which was recently reconstructed,
furnish documentation or certification by qualified person to assure
design
capacity, and establish in writing a regular inspection and maintenance
procedure for slitter systems.
