Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On or about March 1, 1993, through March 7, 1993, at 7508 Clingan Road
in Poland, Ohio, employees were exposed to the hazards of a roof collapse
resulting in serious injuries such as lacerations, broken bones, and/or
death while installing metal plate connected wood trusses approximately
80 feet long on top of 18 feet high masonry bearing walls, in that
inadequate lateral bracing and a lack of top chord diagonal bracing were
used for trusses erected.
The hazards can be abated by, but not limited to, the following:
Establish proper spacing of both lateral bracing and top chord diagonal
bracing by use of a registered professional engineer for trusses over 60
feet in length as recommended by the Truss Plate Institute, and/or by
utilization of truss manufacturer's recommendations during erection of
trusses.

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On or about March 1, 1993, through March 7, 1993, at 7508 Clingan Road
in Poland, Ohio, employees were exposed to the hazards of a truss failure
resulting in serious injuries such as lacerations, broken bones, and/or
death while improperly lifting and installing trusses approximately 80
feet long on to the top of 18 feet high masonry bearing walls with a
spreader bar and only two suspension cables.
The hazards can be abated by, but not limited to, the following:
By utilizing proper lifting practices for truss length over 60 feet per
Truss Plate Institute, or per Gang Nail Warning literature provided with
trusses during delivery.  Lifting instructions call for the use of a
strong
back and/or spreader bar approximately 2/3 to 3/4 of the truss length tied
directly to top chord and web members at approximately 10 foot intervals
to act as a stiffner and to prevent excessive bending of the truss during
lift.
