5(a)(1):  Section 5(a)(1) of the Occupational Safety and Health Act of
1970:  the employer did
not furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees.
Employees involved in the removal of the concrete decking on the north
half of the Lorain
Avenue Bridge over Columbus Road were not provided fall protection
although exposed to fall
hazards of about 23 feet when moving from point to point as well as when
chipping and/or
cutting the concrete and its rebar at any of 12 unguarded "bays",
measuring approximately 7 feet
in width and 12 to 14 feet in length.
Feasible abatement methods may include but are not limited to nets,
scaffolding, catch platforms,
catenary lines with safety belts or other harnessing systems.

adjacent floor
or ground level, were not guarded by a standard railing or the equivalent
on all open sides:
Employees involved in the removal of the concrete decking on the north
half of the Lorain
Avenue Bridge over Columbus Road were exposed to open-sided floors and
potential falls of
about 23 feet when moving from point to point as well as when chipping
and/or cutting the
concrete and its rebar at any of 12 unguarded floor openings, measuring
approximately 7 feet
in width and 12 to 14 feet in length.
