Section 5(a)(1): ANSI B-30.20 1985 Titles: Below the hook lifting devices, Section 20-1.4.4 operational test. Prior to initial use all new, altered, modified, or repaired lifting devices were not tested to ensure compliance with this standard,.../ a) There were no operational tests performed to determine the rated load capacity on three lifting hooks used for lifting a fully assembled 7600 lb. transmission train, using a monorail swivel crane system. This condition exposes the employee to a potential crush hazard resulting from structural failure and/or misuse.
