General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling objects: a) Throughout the plant, wire rope is used for various purposes such as slings, jig hoist guy wires, and hoisting ropes on equipment such as the wheelabrators. Approximately 40of these ropes had wire rope clamps installed with the U-Bolt on the live end of the wire rope instead of the dead end. This could cause the attachment to fail resulting in the load falling causing serious injury to employees. Among other things, one feasible abatement method would be to reverse the incorrect cable clips, and to use the Rigging Manual when working with wire rope.
