Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) On or about August 4, 1989, employees were not protected from the
recognized hazards of fire and explosion. The hazard occurred to
maintenance employees replacing a flare lateral from the Minalk to
the flar header.
General methods of control applicable in these circumstances include,
but are not limited to, the following:
1) Install an additional flar system to handle diverted gases to the
flares.
2) Install bypasses in the header systems so sections could be isolated
for maintenance and the sections could be purged and vented.
