Section 5(a)(1): The issuance of this citition does not constitute a
finding that a violation of the act has occurred unless there is a
failure to contest as provided for in the act or, if contested, unless
the citition is affirmed by the Review Commission.
The violations described in this citation are alleged to have occurred
on or about the day the inspection was made unless otherwise indicated
within the description given below.
On and prior to june 13, 1990, employees on the above sits were subject
ed to the hazards of either being struck by raised lift/vehicle or being
caught between lift/vehicle and the floor resulting in fractures, crush
ing of body parts, multiple internal injuries and/or death due to inad-
vertent activities of the lift while working under a vehicle support by
the lift.
Inadvertent activity or movement of the lifts was more likely due to
lack of or deficiencies in safety equipemnt and procedures as specified
in National Standard Safety Requirements of Construction, Care, and use
of Automobile Lifts, ANSI B-153.1-1981. Among other methods, one feasi-
ble and acceptable method to correct the hazard is to comply with ANSI
B-153.1-1981 along with use of equipment designed in the industry to
meet these standards. Deficiencies noted were as follows:
a. The two in ground, axle engaging lifts on Arlington side were being
used without mechanical devices or safety latches on both fore and aft
posts to hold them in the fully raised position per 3.2.1 of ANSI
B-153.1-1981.
b. The two in ground, frame engaging Globe lifts on Krumroy side were
being operated with bolts of unknown strength substituted for designed
stops in ratches on both sides. Standard 3.2.1 of ANSI B-153.1-1981
specifies a safety factor not less than 3. stops designed for purpose
were substituted during inspection but require fastening to prevent loss
c. The two lifts of a) above were being operated with activating lever
(on floor) that did not return to neutral or off position when released
per 3.1.3 of ANSI B-153.11-1981.
d. For all four in-ground lifts in use there was no regular maintenance
per 6.1 and no periodic inspection per 6.3 of ANSI B-153.1-1981.
