General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Lockout procedures were not practiced when employees placed their hands or stood in a danger zone where unanticipated machinery start up would cause crushing or amputation injuries. Among other methods, one feasible and acceptable abatement method to correct these hazards is to establish and enforce an adequate lockout/ tagout of energy sources procedure. To assist with the essential elements of this lockout/tagout procedure, a copy of 29 CFR 1910.147 "Ccontrol of Hazardous Energy Source (Lockout/Tagout); Final Rule", which becomes effective on December 31, 1989, is attached to this citation.
