Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Fire hazards while conducting improper maintenance procedures which included the replacement of a sight glass on a heater treater with the vessel under pressure and not purged from flammable gases and liquids prior to loosing connections. Among other methods, one feasible and acceptable abatement method to correct this hazard is to establish and enforce sight glass maintenance procedures which shall be carried out by trained personnel. The procedures shall include, but not be limited to the following elements: a) Do not tighten or replace glass while vessel(s) are under pressure. b) Empty and purge vessel(s) containing low flash point liquids or flammable gases before performing maintenance on glass ports.
