Section 5(a)(1) of the Occupational safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
hazard of falling objects:
a) On about August 29,1989, an employee loading a 1974 Ford Gran
onto the bed of a 1969 Chevrolet winch truck located at 120 Trout
Street, Haysville, Kansas, was exposed to the potential hazard of a
falling load.  The 3/4"-diameter wire rope used for hoisting/loading
the car onto the winch truck was not securely attached to the drum.
The wire rope was hooked to the drum with knot. The attachment and
(hook) of the wire rope was secured with aknot and was frayed
reducing the strength of the wire rope.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to:
1) Anchor the drum end of the rope by a clamp, securely attached
to the drum with an arrangement, or its equivalent, provided by the
manufacturer.
2) Install wire rope fittings or properly spliced eye as the wire
rope and attachment.
3) Seize or taper and weld wire rope ends to prevent fraying.
This abatement method is recognized by the industry and wire rop
manufacturers, and recommended by Construction Safety  Associaton of
Ontario, "Rigging Manual" and W.E. Rossenagel, P. E. "Handbook of
Rigging".
