Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) On or about July 13, 1989, employee(s) was exposed to a crush hazard
created by the pushbars in the case storage area of the "Wyard"CPA
800 high speed carton palletizer, serial #14479578 ABCD, in that
while employee(s) was in the process of manually clearing a jam, the
machine inadvertently activated, pinning the employee between the
pushbar and the frame work of the machine.
Among other methods, but not limited to, one feasible and acceptable
abatement method to correct this hazard is to implement and enforce
training procedures as outlined in the manufacturers written safety
procedures for the palletizer and the compans written safety program
regarding machine operation.
When employees are exposed to moving parts of the equipment, such as
clearing jams while positioned on or in the equipment, the equipment
shall be positively de-energized by tagout/lockout procedures to
prevent the inadvertent application of energy or motion to equipment.
