Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) IDENTIFY THE HAZARD: DESCRIBE THE CONDITIONS OR PRACTICES WHICH
ARE CREATING THE HAZARD.  IDENTIFY THE SPECIFIC OPERATION INVOLVED
AND LOCATION. PROVIDE ONE ABATEMENT METHOD WHICH IS ACCEPTABLE TO
CORRECT THE HAZARD.  THE STATEMENT SHOULD READ: AMONG OTHER
METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT
THIS HAZARD IS..."

a) The hazard of amputation of fingers and hands while operating the
Darley hydraulic
press in that set-up/press brake operators were unfamiliar with the
operation
and set-up
of presence sensing devices used to safe guard employees while operating
the Darley
hydraulic press, and were unfamiliar with the availability, function and
function and
purpose of setting #2 on the selector switch of the press which would
allow the press to
operate using a two hand/foot follow through stroke.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is: train employees in the safe operation of machinery including
the function and
operation of all press controls, and the function, operation, limitations,
and time distance
factors associated with the presence sensing devices.
