Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to the hazard of falls from elevated work
stations while suspended in a single-point suspension scaffolds:
In that, on or about 2/2/94, an employee was working from a
singlepoint
suspension scaffold suspended (rigged) from a structural
steel beam of the building structure.  The scaffold loadline and
employee's lifeline had been rigged to the same structural steel
beam.  Among others, one feasible and acceptable method to
correct/abate this hazard is to follow industry and scaffold
manufacturers' guidelines such as but not limited to Scaffold
Industry Association, Inc, Code of Safe Practices for Suspended
Powered Scaffolds (Form 0108S); formally (Form 0104S), paragraph
F1, which is contained in the Operators/ Safety Manual of
instructions furnished by the scaffold manufacturer.  "Lifeline(s)
