Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazards of broken bones, amputations and death while operating a freight/passenger elevator which lacked a interior door and interlock switch to protect the employees from the shear points created as the car moved up and down the shaft. One feasible and acceptable method to abate this hazard is to install protective inter car doors with micro switches to prevent the car from operating unless the door is closed. After completion of all such work the elevator should be inspected and certified as required by Ohio state law.
