General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. SALT CITY COGENERATION PROJECT: Boiler House, coal handling area, 5th. floor; employees were exposed to the hazard of being caught in the coal conveyor belt system when started due to the fact that the coal transfer area is not visible from the operators start up position. Among other methods, one feasible and acceptable abate- ment method to correct this hazard is to install an audible and/or visual warning device to alert employees that the conveyor is about to start. The warning device shall be actuated by the controller device starting the conveyor and shall continue for a required period of time before the conveyor starts as referenced in ANSI B-20.1- 1990, section 5.11.2 (b)(1) - Control Stations.
