employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the load
slip off the hook, or the
lifting device fails:
a)Production/Pouring Area, on or about 12/19/97: A CM 1 ton overhead
electric chain hoist being used to move a cement hopper.  The hoist had the
hook throat latch missing from the load hook.
b)  Production/Pouring Area, on or about 12/19/97: A CM 2 ton overhead
electric chain hoist being used to move forms with the concrete poured in
them.  The hoist had the hook throat latch missing from the load hook.
c)Storage Room, on or about 12/19/97:  A CM 1/4 ton overhead electric chain
hoist being used to move small vault lids.  The hoist had the hook throat
latch missing from the load hook.
d)Storage Room, on or about 12/19/97:  Three CM 1 ton overhead electric
chain hoists being used to move vault lids with liners.  The hoists had the
hook throat latches missing from all the load hooks.
ABATEMENT NOTES:
Among other methods one feasible and acceptable method to correct this
hazard is
to:
1.Install and/or repair hook throat latches.
2. Follow the requirements of the American National Standard for electric
chain
hoists (ANSI/ASME HST-1M-1989)
Written abatement verification is required for this citation.
