Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  the
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to fall hazards:
(a)  An employee at about 12 feet above ground level was
standing on a pipe while applying insulation on another pipe.
There was no fall protective equipment being used to prevent
the employee from accidentally falling to the ground and/or
to other equipment directly below him, on or about 10/30/91.
Among other methods, one feasible and acceptable method to
correct this hazard is the proper use of safety belt and
lanyard.
