General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of falling from an unsecured metal ladder onto objects and
surfaces below:
The outside wall of the Shipping Department is where a
20-foot metal extension ladder was used without being
secured, on or about 6/11/90.
Among others, some feasible and acceptable abatement methods to correct
this hazard are: (1) properly secure the metal ladder to prevent its
being dislodged from its intended position, (2) use a more stable
apparatus to perform maintenance at elevated levels, i.e., a scaffold,
(3) enforce safe maintenance procedures, and (4) do not push or pull
when working from an unsecured metal ladder.
