Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employee(s) in that
employee(s) were exposed to the hazard of being struck by equipment
such as in slag granule processing:
(a)   Workplace - The FIAT ALLIS FR-15, front-end loader, was
operated with an inoperable back-up alarm while moving
smelter furnace slag to load a hopper for the drying and
processing plant, on or about 7/7/93.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to always inspect the equipment prior to
use and correct any deficiencies prior to operation.
