Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of the uncontrolled release of energy and hazardous materials: a)At the establishment - Wall thicknesses were not determined before installing, repairing, or replacing the overlay on batch digesters Numbers 2, 3, 5, 6, 8, 9, 12, and 13, on or about 5/4/94. Among other methods, one feasible and acceptable method to correct this hazard is to implement an in-service pressure vessel inspection program in accordance with the provisions outlined in the National Board Inspection Code (NBIC-ANSI/NB-23), Chapter V. b)At the establishment - Adequate progressive inspection and maintenance records were not maintained for the locations and thicknesses of monitor samples and other critical inspection locations of batch digesters Numbers 2, 3, 5 through 10, 12, and 13, on or about 5/4/94. Among other methods, one feasible and acceptable method to correct this hazard is to implement a recordkeeping program in accordance with the provisions outlined in the National Board Inspection Code (NBIC-ANSI/NB-23), Chapter V, Paragraph U-104.aph
