Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to:
a) Galvanizing Dept. - the basement area underneath the hot metal kettle
had not been identified as a confined space by the employer.
Maintenance employee as allowed to enter a manhole to go under the
floor and check the air to gas ratio on the Kettle burners. Safe
entry procedures for a confined space were not followed.
Among other methods, feasible and acceptable means to correct this
hazard include:
1) Develop an entry "permit" system.
2) Develop a qualified person to perform atmosphere testing prior to
entry and during entry.
3) Establish a "buddy" system so that an attendant is available.
