Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)Korean store construction site, main road at Futiga: On November 1, 2006, operator of a front end loader/backhoe equipment was operating the equipment to dig trenches and move dirt. Regular rebars were modified and used for pins to secure the bucket or the working shovel of the equipment instead of pins provided by the manufacturer. Slippage of the coupling could result in unintended separation of the bucket during operation or maintenance, potentially striking or injuring an employee. Among other methods, one feasible and acceptable abatement methods to correct this hazard is to follow the manufacturer's operation manual and reinstall all fasteners with the same part number.
