Section 5 (a) (1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to injuries due to separation of metal parts under
pressure on the
hot water storage tanks and burns from the hot water and steam:
a)Boiler Room - On or about April 25, 2008, and at times prior thereto,
pressurized hot water was stored in non approved storage tanks which were
not recognized by the American Society of Mechanical Engineers (ASME)
VIII, Division 1 (Boiler and Pressure Vessel Code).
AMONG OTHER MEHODS, ONE FEASIBLE AND ACCEPTABLE
METHOD TO CORRECT THE HAZARD:  Use hot water storage tanks
certified in accordance with American Society of Mechanical Engineers
(ASME) VIII, Division 1 (Boiler and Pressure Vessel Code) and marked in
accordance with paragraph UG-116 of this document.
b)Boiler Room - On or about April 25, 2008, and at times prior thereto,
the hot
water boiler providing hot water and steam to hot water storage tanks for
plant
use did not receive inspections to verify the boilers were safe to operate.
AMONG OTHER MEHODS, ONE FEASIBLE AND ACCEPTABLE
METHOD TO CORRECT THE HAZARD:  Inspect boilers by a certified
boiler inspector in accordance with the National Board Inspection Code for
Boilers and Pressure Vessels Part 2, Section 1 Paragraph 1.5; and Section
2,
paragraph 2.2.. NOTE:  Inspection frequency shall be determined in
accordance with the National Board Inspection Code for Boilers and Pressure
Vessels Part 2, paragraph 4.4.
c)Boiler Room - On or about April 25, 2008, and at times prior thereto,
the hot
water storage tanks (unfired pressure vessels) did not receive inspections
to
verify the storage tanks were safe to store pressurized hot water and
steam.
AMONG OTHER MEHODS, ONE FEASIBLE AND ACCEPTABLE
METHOD TO CORRECT THE HAZARD:  Inspect hot water storage tanks
by a certified boiler inspector in accordance with the National Board
Inspection Code for Boilers and Pressure Vessels Part 2, Section 1
Paragraph
1.5; and Section 2, paragraph 2.3..
d)Boiler Room - On or about April 25, 2008, and at times prior thereto,
the hot
water storage tanks and boiler piping did not receive inspections to
verify the
piping was safe for the pressurized hot water and steam.  The employer
utilized non-ferrous (galvanized) piping connected to a ferrous metal hot
water
storage tank in a corrosive environment; thereby, creating accelerated
galvanic
corrosion. The piping connection on one storage tank was so corroded that
it
was held up by dimensional lumber.AMONG OTHER MEHODS, ONE FEASIBLE AND
ACCEPTABLE
METHOD TO CORRECT THE HAZARD:  Inspect piping systems by a
certified boiler inspector in accordance with the National Board Inspection
Code for Boilers and Pressure Vessels Part 2, Section 1 Paragraph 1.5; and
Section 2, paragraph 2.4..
