Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  the
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of being struck by the heavy steel sides of
concrete forms:
a)  Yard - In the course of the work year, as much as several
times weekly when working, employees are exposed to the hazard
of being struck by the sides of heavy steel forms which have
been opened and kept from falling outward by lengths of 2X4's
could be easily dislodged or even give way.
Among other methods, one feasible and acceptable abatement
method to correct this would be the use of heavy steel chains
attached at the corners to keep the heavy steel sides of the
forms from falling out.
