Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
(a)  The hazard of unanticipated contact with heated and/or pressurized
acids or gases by employees, subcontractors, or others from outside
agencies during an emergency, such as firemen, who may mistakenly actuate
valves or disconnect pipes at the wrong time or place, as none of the pipes
in the facility were labeled or color coded, on or about 12/12/91.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to follow directions given in ANSI A 13.1-1981
Scheme for the Identification of Piping Systems.
(1)  Positive identification of the contents of a piping system shall be
by lettered legend giving the name of the contents in full or abbreviated
form, with additional details such as temperature, pressure, etc.  Arrows
whall be used to indicate direction of flow.
(2)  Color may be used to identify the characteristic properties of the
contents, but its use shall be in combination with legend.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to the hazard of over presssurizing steam turbines leading to explosion
of the casings or components:
(a)  Sulfuric Acid Plant, between the Cooling Basin and the Cooling Tower,
two steam turbines used to run the "BJ" pumps did not have a full flow
relief valve installed in the exhaust line between the turbine casing and
the first shut off valve, on or about 10/8/91.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to install full flow relief valves on all steam
turbines.
(b)  Sulfuric Acid Plant, steam boiler water feed pump, the sentinel relief
valve was removed from the turbine and not replaced, on or about 10/22/91.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to install sentinel relief on all steam turbines.
(c)  Sulfuric Acid Plant, preventive maintenance and testing was not
conducted to ensure that the sentinel relief valves on steam turbines were
functioning properly, on or about 10/8/91.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to develop inspection procedures for maintenance
of the sentinel relief valves, according to the steam turbine manufacturers
operating manuals.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employement and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of falls of over 20 feet:
(a)  The weight of the personnel platform was not posted and the maximum
intended load permitted in the personnel platform was also not posted.
The personnel platform, the use of which is normally prohibited, except
when the erection, use, and dismatling of conventional means of reaching
the work location, such as by a ladder, stairway, aerial lift or scaffold,
would be more hazardous or is not possible because of work conditions,
was suspended from the load line of the link belt truck crane HTC-850 in
the Sulphuric Acid Plant, on or about 10/25/91.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is that the personnel platform shall be conspicuously
posted with a plate or other permanent marking which indicated the weight
of the platform and its rated load capacity or maximum intended load.
(b)  Trial lifts, when made, were not done with the personnel platform
loaded at least to the anticipated liftweight.  The personnel platform,
the use of which is normally prohibited, except when the erection, use
and dismantling of conventional means of reaching the work location, such
as by a ladder, stairway, aerial lift or scaffold would be more hazardous,
or is not possible because of worksite conditions, was suspended from the
load line of the Link Belt truck crane HTC-850, in the Sulphuric Acid
Plant.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to make a trial lift with the unoccupied platform
loaded at least to the anticipated liftweight from the location where
employees would enter the platform to each location at which the personnel
platform was to be hoisted and positioned.
