platform.
a)  At the construction site - Employees were exposed to fall hazards.  A
forklift tractor with
an unapproved platform was used for lifting personnel to perform work,
access and/or egress
higher elevations, on or about 3/21/00.
b)  At the construction site - Employees were allowed to ride on an
unapproved platform
placed over the forks of a forklift tractor, while this equipment was
traveling, on or about
03/21/00.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is:
1. Provide an approved platform designed for this purpose, securely
attached to the forks.
2. Prohibit the practice of riding on this platform while the forklift
tractor is traveling.
