Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushed foot injury: a) 2CM line- The lift platform was not equipped with a slanted toe guard, skirting or safe toe clearance as outlined in Commercial Standard 202-56, thereby subjecting the employee to a foot injury. GENERAL METHODS OF CONTROL APPLICABLE IN THESE CIRCUMSTANCES INCLUDE, BUT ARE NOT LIMITED TO, THE FOLLOWING: 1) Modify the lifting aid to be in compliance with Commercial Standard 202-56.d

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential violent reaction: a) Warehouse - Flammable storage room- 55 gallon drums of glacial acrylic acid were stored with 30 gallon drums of organic peroxide. GENERAL METHODS OF CONTROL APPLICABLE IN THESE CIRCUMSTANCES INCLUDE, BUT ARE NOT LIMITED TO, THE FOLLOWING: 1) Provide for separate storage of the two incompatible chemicals. 2) Use signs to designate separate storage areas and train the employees in the potential hazard of the inadvertent mixing of the substances. 3) Provide drainage basins to prevent the inadvertent mixing of the substances.
