Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of a fall of approximately 36 feet or 3 floors:
a)  Employees could access the elevator shaft when the car
was not on the landing, by holding the south bi-door shut and
operating the up/down steel cable to call the elevator car
to their landing.
Among other methods, one feasible and acceptable abate method
to correct the hazard is, install hoistway door interlocks
that allow the doors to open only when the elevator car is in
the landing zone.  This requirement can be found in the
following consensus standards:
1.  American Society of Mechani al Engineers publication
A17.3-1990 titled, "Safety Code for Existing Elevators and
Escalators".  The specific referenced portion is "Part II
Hoistways and Related Cosntruction for Electric Elevators,
Section 2.7, Hoistway Door Locking Devices, Parking Devices,
and Access".
or in the alternative;
2.  The same requirement for door interlocks, to insure that
an elevator car be in the landing zone before the doors can
be opened, is found in "American National Standard Institute
Safety Code for Elevators, Dumbwaiters and Moving Walks
A17.1-1971, Part I titled "Hoistways, Hoistway Enclosures and
Related Construction for Electric Elevators, Section III
Hoistway-Door Locking Devices, Car Door or Gate Electric
Contacts, Hoistway Access Switches, and Elevator Parking
Devices.  Rule 111.1 Locking Devices Required".
