Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe crushing injuries caused by falling or swinging loads released by one or more hoists due to failure of the hoist hook or brake, overloading of the hoist support system, or improper lift coordination during multi-hoist lifts: (a) To lift railroad car bolsters with side frames weighing 7,400 pounds, two chain hoists of one-ton capacity each and one electric hoist of two-ton capacity suspended from an A-frame structure were used. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the criteria in ANSI (American National Standards Institute) B30.11, 1973: Monorail Systems and Underhung Cranes. (1) The rated load capacity for the trolley beam and support members must be determined and marked on the structure. (2) Conduct regular monthly inspections with records to determine the condition of the brakes, chains, hooks, top stop limiting device and other components. When any defects are found, the hoist must be taken out of service until repaired. (3) When two or more crane hoists are used to lift a load, assign a qualified responsible person to coordinate the lift so that at no time will any hoist be subject to a load exceeding its rated capacity.
