Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Hazards such as, but not limited to, fires, explosions, toxic substances, and potential valve failures due to overheating or over pressurization and extreme leakage of injection valves during on line stream repairs of various processes on or about November 18, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Use a positive seal ball valve that is rated for the pressure, temperature and is compatible with the service you are working with and/or, 2) If a positive seal ball valve is not available for your particular situation, then the next option is to preseal the injection valve shutoff to the drill through as stated in Team's Safety Bulletin #61.
