Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On 8/12/98, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of exposure to ammonia: (a) where refrigeration systems, Mycom and F.E.S., did not have a preventative maintenance program implemented to ensure that the systems were in proper working order. An acceptable means of abatement would be to adopt the inspection and maintenance schedule described in IIAR Bulletin 110-Guidelines for Start-Up Inspection and Maintenance of Ammonia Mechanical Refrigeration Systems or adopt the maintenance schedule provided by the manufacturers. NOTE: IIAR Bulletin 109, IIAR Minimum Safety Criteria for a Safe Ammonia Refrigeration System, provides information for equipment, frequency of safety inspections and inspection checklists.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On 8/12/98, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of exposure to ammonia: (a) where emergency respirators (self contained breathing apparatus) were not located outside and close to exit doors of compressor rooms. All respirators (SCBAs) were located in a storage building more than 100 yards from compressor rooms. An acceptable means of abatement would be to locate one or more self contained breathing apparatus directly outside the machine room doors. NOTE: IIAR Bulletin 109, IIAR Minimum Safety Criteria for a Safe Ammonia Refrigeration System, provides information for General Ammonia Refrigeration System Safety Requirements in Section 4.10.on
